There is a consolidated relation between malnutrition and HIV/AIDS, which is enduring and perpetuating the immunodeficiency of patients on highly active antiretroviral therapy (HAART), with or without opportunistic infections (OI). Nevertheless, there are still big gaps in knowledge of the effects of food intake on imunological response, as well as on the effects some specific nutrients and suplements might have in reducing morbidity and mortality in people living with HIV/AIDS (PLHA). This study aims at investigating possible correlations between nutritional habits and status, and treatment failure and consequent changes in the HAART regimen.
This is a cross-sectional study on PLHA out patients attending a well-known Hospital in Rio de Janeiro for treatment and care of this specific population. We conducted a statistical analysis to explore the prevalence ratio (PR) of possible associations between variables such as treatment failure, body mass index, food consumption, dyslipidemia, immunological failure and OI. Results: From the 49 patients, 57% were males, with an average age of 46 years old, and 22% showed treatment failure. Out of the explored associations, the statistically relevant ones were: treatment failure and high levels of triglycerides (>150mg/mL) with PR of 6.6 (CI 1.01-49.84; p-value < 0.02), and treatment failure and obesity with PR of 12.7 (CI 1.07-176.9; p-value < 0.008). Another interesting finding that deserves further investigation is the daily consumption of bananas that seems to have different intake rates between patients with or without treatment failure. The difference between the average banana intake amounts by the two groups was -1.23 (CI -2.37-0.096; p-value < 0.0342), with the greater consumption rate found in the group with treatment failure. |

A correlação entre má-nutrição e HIV/AIDS já foi amplamente demonstrada e contribui para a perpetuação da imunodeficiência nos pacientes em uso de terapia antirretroviral (TARV). Ainda não existe total esclarecimento sobre a influência da ingestão alimentar na resposta imunológica nos pacientes em uso de TARV. Uma pobre resposta imunológica nesses pacientes pode levar não só à falha imunológica, mas também clínica, terapêutica e viral com consequente risco de progressão da doença para AIDS, infecções oportunistas e a necessidade de utilizar segundas linhas terapêuticas. Este estudo visa investigar a possível correlação entre hábitos alimentares e estado nutricional das pessoas vivendo com HVI/AIDS, com a falha terapêutica e a troca de TARV. Para isso foi conduzido um estudo transversal em pacientes atendidos em um Hospital Federal de referência no Rio de Janeiro. Resultados: Dos 49 indivíduos avaliados, 57% do sexo masculino, com idade média de 46 anos, 22% apresentaram falha terapêutica. Dentre as associações exploradas, parecem, a princípio, significativas a razão de prevalência das variáveis falha terapêutica e hipertrigliceridemia (>150mg/mL) de razão de prevalência (RP) de 6,6 com um intervalo de confiança (IC) de 1,01-49,84 (p-valor < 0,02); a RP de falha terapêutica e obesidade também significativa sendo 12,7, e o IC, 1,07-176,9 (p-valor < 0,008). Outro dado interessante a ser mais investigado é o consumo diário de banana, a fruta mais consumida pelos pacientes, que parece ter consumo diferente entre pacientes com ou sem a falha terapêutica sendo a diferença entre os dois grupos na média do consumo de bananas -1,23, e o IC entre -2,37 e -0,096 (p-valor < 0,0342) com consumo maior no grupo que fez falha terapêutica. |
