dc.description.abstracten Introduction: Charcot-Marie-Tooth disease (CMT) is a common hereditary peripheral neuropathy, prevalence of 1:2.500, with heterogeneous clinical and genetic manifestations. Distal weakness and sensory symptoms, usually beginning in the lower limbs, are common to mutations in more than 130 identified genes. The age at onset, severity, mode of inheritance, and neurophysiological findings help classify the different forms of the disease; however, there is wide phenotypic variability, even among individuals carrying the same mutation and within the same affected family. Objective: To describe the clinical and neurophysiological characteristics of patients with CMT followed at two reference centers in Rio de Janeiro. Methods: Ambispective study including patients with genetically confirmed diagnoses. Medical records from 2018 to 2025 were reviewed, and clinical, functional (CMTNSv2, NIS LL), and neurophysiological data were collected. Results: Seventy-two patients were evaluated; 56 from 44 families had genetic confirmation. PMP22 duplication was the most frequent variant (52.5%), followed by PMP22 deletion (12.5%), MPZ (10%), MFN2 (5%), GJB1 (5%), MME (1, 2.5%), PMP22 point mutation (1, 2.5%), MORC2 (1, 2.5%), PLEKHG5 (1, 2.5%), LRSAM1 (1, 2.5%), and HSPB1 (1, 2.5%). The novel PLEKHG5 p.Arg20Gln variant may be associated with recessive intermediate CMT. Nine patients had no identified mutation, and seven presented variants of uncertain significance. Discussion: The clinical and neurophysiological findings in this cohort are consistent with those described in the literature, showing a predominance of demyelinating phenotypes associated with PMP22 duplication, the main cause of CMT1A. The distribution of mutated genes reflects the expected pattern in large cohorts, with smaller but relevant contributions from MPZ, MFN2, and GJB1. The identification of a novel variant in PLEKHG5 (Arg20Gln) expands the genotypic spectrum described for recessive intermediate CMT. The presence of cases with variants of uncertain significance or without confirmed genetic diagnosis highlights the remaining diagnostic challenges, including technical limitations and restricted access to broader genetic testing. Conclusion: Epidemiological studies on Charcot Marie-Tooth disease remain limited in Brazil. This study reinforces the marked clinical and genetic heterogeneity of the disease and underscores the importance of national cohort descriptions to enhance the understanding of genotype-phenotype correlations, guide diagnostic approaches, and support the management of hereditary neuropathies. pt_BR
